Forms to Finalize Registration
Before the first day of school, please download, complete, and submit the following forms either by mail or in person:

Health/Doctor Release Form
(must be completed by a licensed physician,
physician’s assistant, or nurse practitioner)

Child's Confidential History Form
Tuition Payment Information
Tuition is due on the 1st of each month, beginning September 1st and ending June 1st. Deposits are non-refundable and will be applied to June’s payment.​​​​​

PAYING BY CHECK:
You will receive a book of nine (9) Payment Coupons.
One (1) coupon must be included with each monthly payment, due on the 1st of the month.
